Case Study Rewards Program
All participants will be awarded $500.00 Cdn

Who is eligible to participate in the Rewards Program:
This program is open to Occupational Therapists, Physiotherapists, and others who provide
seating evaluation or delivery services to wheelchair users. Just follow a few easy steps
(see a case study sample form online)
STEP 1. Get pre-approved. It’s simple, fast and easy to do. Just complete
and submit the online pre-approval form.
STEP 2. You will be contacted by an HPU representative to confirm
your pre-approval status.
STEP 3. You will be sent a case study form for completion.
STEP 4. Select a client for the case study. Evaluate the client in their old
equipment, provide new seat component and re-evaluate with the new equipment.
STEP 5. Enter the information on the evaluation form (including photographs)
and submit the evaluation form with photo release and client photos/pressure mapping images to HPU Rehab.
